Thanks, Paul.  It looks like I need to update the transform that generates
the attlists.  When an attribute name is shared by several elements but has
different description, I have to catch each such case and generate a conref
id by combining the element name and the attribute name. I'll make sure the
ID conrefs are linking to the right descriptions.

I just realized that I have not added the @start and @end descriptions for
index markup conrefs--that's new since the last drafts.

Throughout this language spec, the data type of
the "id" attribute is listed as "ID".  Isn't
that a mistake?
