The XSL stylesheets are currently set up to respond
only to role="bold" (semantic markup only by
a stretch, IMHO).  Since the definitive guide
mentions using either "bold" or "strong", and
since "strong" is more semantic, I think
the stylesheets should probably support both.  But
if you want bold today, you either have to change
your attribute values to "bold" or add this
customization to your processing:
