This is a bug in libxml (used by xsltproc). It doesn't allow namespace
declarations to inherit across entity boundaries. Daniel concedes that
it's a bug, though I think he is of the opinion that the spec is
broken in this regard.

I've worked around this, and fixed a few uglier bugs, in the last
couple of days, but my network connection has been down. It's back
now, so expect a new release this evening.
