So am I. You can override xsl:output just like you can anything else. 
Here's an edited version of the wrapper stylesheet I use in one 
environment:

<xsl:stylesheet version='1.0'
   xmlns:xsl="http://www.w3.org/1999/XSL/Transform";
   xmlns="http://www.w3.org/1999/xhtml";>

   <xsl:import href="/path/to/xhtml/docbook.xsl"/>

   <xsl:include href="local-defs.xsl"/>

   <xsl:output method="xml"
     encoding="ISO-8859-1"
     indent="yes"
     doctype-public="-//W3C//DTD XHTML 1.0 Transitional//EN"
     doctype-system="http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd"/>

   ...
</xsl:stylesheet>
