2.2.1 Line 413. Add to warn reader not to equate Initiator with Sender (we need this to help the reader at least)

Note that an Initiator MSH need not be a Sending MSH if, for example, the message unit is an ebMS signal message unit; in fact, the Initiator MSH can be a Receiving MSH when operating in pull mode.
