* filename and version number as above

* headers & footers, publication date, etc. verified.

* Line numbering turned on

* "Fast Saves" turned off
   in Word..
     Tools > Options > "Save" Tab
       Allow fast saves                   -- unchecked

* all changes accepted
   in Word..
     Tools > Track changes > Accept or Reject Changes... >
       [follow directions to accept all changes (as necessary and appropriate)]


* Track Changes turned off
   in Word..
     Tools > Track changes > Highlight Changes >
       Track changes while editing          -- unchecked
         Highlight changes on screen        -- unchecked
         Highlight changes in printed doc   -- unchecked

* Please include .pdf if you can
