First draft of survey

Ok dear LwDITA/FINCH/MAGMA2/LITA sub-comittee friends,

Following Jang’s recommendation, the survey has two branches based on the participant’s knowledge/awareness of DITA. There are gaps in language and terminology, of course, but that happens in first drafts.
