More re LwDITA and specialization

Currently, the LwDITA DTDs only uses entities for <ph> and <data>. This means that anyone who wants to specialize XDITA, can only create specializations of <ph> and <data>. The same would be true for constraints and expansion.

Was this an intentional choice?
