Spec Comment: 2.1.4.4.2 Constraint module XSD coding requirements:Clarify coding detail

Should add a non-normative note pointing out that any <xs:redefine> requires
removing the equivalent xs:include from the document type shell.

For example, if you include a module that redefines elements from
topicMod.xsd you must remove the inclusion of topicMod from the shell
schema.

This is a standard XSD rule but is not an obvious rule.
