Proposal for adding the rev attribute to title

To the DITA TC 

I have a requirement to add the rev attribute to title so that changes to a title can be flagged for reviewers and regulatory agencies. If there are other areas that don't have the rev attribute and should, these might be added to this Stage 1 proposal.
