Proposal 13027: Draft comment and required cleanup everywhere

I have posted an updated version of this proposal, as well as a package of
vocabulary module declaration changes for DTD, XSD, RNG, and RNC. I would
like to get this proposal on the agenda for promotion to Phase 3.
