Re: DOCBOOK-APPS: Re: XSL: fix xref to question

The second problem hasn't been fixed as of version 1.15 of the XSL
stylesheets.  The xref hyperlink doesn't match the question anchor.  I'm
attaching a small sample that illustrates the problem.
