DOCBOOK-APPS: Tables questions

I'm using DocBook XML v4.1.2 and Norm's stylesheets v1.57:

Is there a way to prevent tables from spanning a pagebreak
in the print version of the stylesheet ?  Alternatively, 
how can I force a pagebreak ?

Is there a way to control the table width, pgwide seems 
to have no effect - again for the print version ?
Tables always seem to span the entire width of the page 
leaving lots of white space around the text in the columns.
