DOCBOOK-APPS: Saxon 6.1 is unsupported at the moment

Several people have pointed out that the extension functions in
the DocBook XSL Stylesheets don't work with Saxon 6.1. That's true.
They don't. I plan to support 6.1 "real soon now", but in the meantime,
please use an earlier version or turn off the saxon.extensions.
