DOCBOOK-APPS: Automatic TOC with DocBook fo xsl ?

Hi,

I'm using DocBook XSL stylesheets v1.18 and somehow it does not seem to
generate a TOC with the fo stylesheets ?
Is there a paremeter to set ?
Also, I can't seem to find any entry regarding Page format (A4 vs. Letter).
One last thing, I want to customize the way the page number is displayed.
What is the template used for that ?

Can anyone please help ?
