DOCBOOK-APPS: TOC Level 1 entries need keep-with-next?: #t by thelooks in DSSSL

Hi,

It seems that a level 1 TOC entry generated by dbautoc.dsl should have the
keep-with-next? attribute set to #t.

I added to the make-paragraph:

    keep-with-toc?: (if (= level 1) #t #f)

Now to see if this works ...
