DOCBOOK-APPS: table of contents problem

Hi Everyone,
I'm using DocBook 4.1 with the LDP.dsl for a book, and I'm having a problem
with headings showing up in my table of contents.  Headings that are tagged
with <sect3> do not show up in the chapter table of contents unless there
is more than one of them in that particular <sect2>.  I was wondering if
anyone else has had this problem or knows how to fix it.  I appreciate the
help!
