DOCBOOK-APPS: PDF toc depth

Title: PDF toc depth

I am using Passivetex to create PDF output. For any reason, the depth of the PDF toc seems to be limited to 3 levels. The docbook toc shows correctly all 5 hierarchies of my document.

Question 1: Does anybody know a way to increase the hierarchy levels for the PDF toc output ?

Question 2: The first level of my PDF toc output shows the book title. But I would like to start with the chapter titles at the first level.
