DOCBOOK-APPS: New HTML Help parameter htmlhelp.hhc.show.root

Hi,

there is a new parameter in CVS version of XSL stylesheets. If you set
htmlhelp.hhc.show.root to 0, there will be no entry for the root element
in ToC in a left pane. This means that user will see titles of nested
elements (usualy chapters) directly.
