DOCBOOK-APPS: Multiple Bugs in 1.49

Found a couple of bugs in XSL 1.49:


- Footnotes in HTML sheets is generated twice:

   [1] [1] some footnote

   Maybe this is related to extended memory requirements when translating 
   a large document to HTML. 


- Bugs in FO implemenation of emphasis with role="bold".

  Text is generated twice (first in italic, followed by same text in bold)
