I can confirm the following bugs are solved:

- Recoverable error
  at xsl:attribute on line 148 of
  +file:/home/mw/docbook-xsl-1.52.1/html/chunker.xsl:
  Cannot write an attribute node when no element start tag is open

  DOCTYPE inserted correctly.


- Only the first indexentry starting with a '\' included in the index.
