I don't see anything in your customization layer that looks like it
would help. And I agree, FOP from CVS this morning doesn't handle your
table.

I'm suspicious that these messages

bp==null  cls=0
bp==null  cls=4
bp==null  cls=0
bp==null  cls=4
bp==null  cls=0
bp==null  cls=4
bp==null  cls=0
bp==null  cls=4

indicate deep badness.
