The most likely reason is a bug in Xalan. Xalan has almost always 
problems with complex DocBook XSLs. Version 1.60.1 works at least with 
Saxon and xsltproc. I think that Xalan 2.4 works also. Probably some the 
bug is related to changes between 2.4 and 2.5. I would recommend you to 
report bug to Xalan developers.
