xsltproc: fox:outline element missing for TOC bookmark in PDF

I had no reply to an earlier post on this subject:

Since I still have the same problem, I insist and try again ;)

Now, I further noticed that the problem is only for books that *do not have*
a glossary, an appendix or a bibliography at the end.

For books ending with one of the above components, I get the TOC bookmark in the 
AcroRead bookmarks window, right after/under the book title bookmark, as 
expected.

In our collection we have many more rather "simple" documents than "complex" 
ones, that's why I am still looking for a fix that works for the majority of our 
books...
