body-start() = the value of the start-indent +
               start-intrusion-adjustment + the value of the
               provisional-distance-between-starts of the closest
               ancestor fo:list-block

Outside of an fo:list-block it's presumably:

body-start() = the value of the start-indent +
               start-intrusion-adjustment
