revhistory & proportional-column-width

A while back, you posted a message pointing out that handling of
revhistory in FO output seems to be broken.

I'm wondering what needs to be done to fix it. It's be nice to have the
option of it being hard-coded at 100% width, but if that's not doable in
the short term, just setting it at 100% to correct the problem for now
would seem to be the right thing to do.
