Generating webhelp with applethelp.xsl

I'm attempting to generate Webhelp using the
applethelp.xsl stylesheet from sourceforge
(http://cvs.sourceforge.net/viewcvs.py/docbook/contrib/xsl/applethelp/),
but I keep getting the following error:

Error at saxon:output on line 337 of
http://docbook.sourceforge.net/release/xsl/1.64.1/html/chunker.xsl:
  Failed to create output file files/ch01s02.html

I've added the new htmlhelp-common.xsl customization
as well.  Any ideas or references?
