Yes, it is. Modify the helpidx.content template in javahelp.xsl. Change this line:

<xsl:apply-templates select="//indexterm" mode="idx"/>

to this: 

<xsl:apply-templates select="//indexterm" mode="idx">
 <xsl:sort select="primary"/>
 <xsl:sort select="secondary"/>
 <xsl:sort select="tertiary"/>
</xsl:apply-templates>
