Please note that fo:wrapper is also used in other places and may be not 
all of them can be simply changed to fo:block or fo:inline.

As I recall this problem is really not new. See code in fo/index.xsl:

<xsl:template match="indexterm" name="indexterm">
   <!-- Temporal workaround for bug in AXF -->
   <xsl:variable name="wrapper.name">
     <xsl:choose>
       <xsl:when test="$axf.extensions != 0">fo:block</xsl:when>
       <xsl:otherwise>fo:wrapper</xsl:otherwise>
     </xsl:choose>
   </xsl:variable>
   ...

Now it seems that it is quite unfair to call it AXF bug :-)
