Re: [docbook-apps] Re: indexterm indentation

I agree that the hardcoded start-indents should not be in there for
secondary and tertiary index entries.  This will require more changes than I
have time to do before the next stylesheet release.  I think there should be
a new parameter for the index indent increment, similar to the
'toc.indent.width' parameter.
