Probaly partly solved: If I do add an id attribute to the top-level 
<book> element (*before* I run make-toc.xsl), then the ToC has the 
filename "bk01-toc.html", when using chunk.tocs.and.lots and chunk.toc.

The name "bk01-toc.html" is still pretty ugly, at least as a permanent 
link, and I haven't found a way to override it.
