insert a pagebreak / a picture in header / pagenumbers /Table ofContents

I'm trying to work with
DocBook to create some Manuals and put them out as PDF, RTF and HTML.
Working with DocBook XML and XSL some questions accrued. I also RTFM,
but not as yet I couldn't find the answers.

I hope I get  answers in this
list which helps me

1. How can I insert a pagebreak after or before a section in my
Simplify DocBook so that converting it to PDF or RTF the pagebreaks are
made?

2. How can I insert a picture to the page-header?

3. How can I change the showing pagenumber to this: "Page 1 of 3"

4. How can I change the "Table of Contents" to another word, e.g. in
german: "Inhaltsverzeichnis"?
