I found the problem.  The version of xsltproc I was running
was very old:

    Using libxml 20510, libxslt 10033 and libexslt 722

When I ran it against a newer version, it worked fine:

    Using libxml 20616, libxslt 10112 and libexslt 810

Thank you VERY much for helping me eliminate all the other
links in the toolchain.  After reading your last e-mail,
I realized that it pretty much had to be xsltproc.
