Finishing a book

Hello list,

I am about to finish my book and I ask my self (and the list archive)  
how to make a cover with first page and last page.

I read that DocBook 5.0 will have a cover stylesheet but... Is there  
a solution with "old docbook" ?

Did you made your cover with Docbook and secrets formulas or did you  
made it with something else (like Illustrator) ?
