db5 db.cals.table w/ db-xsl-1 generates double caption

I have an xsl 1.0 html output test case that generates a bold caption
above the table and a normal caption below the table when the XML
source conforms to the db.cals.table from DocBook 5. This does not
happen in the xml->fo->pdf case, where only the bold caption above the
table appears.
