How to get emphasis with both Bold and Italic, in FO?

I have some text I'd like to see as both bold and italic, e.g. 
"bold-italic". 
I know <emphasis role="bold">, and <emphasis role="italic"> doesn't 
exist, but is the default

How do I do that in FO?
