I believe this problem was fixed in version 1.71.1 
of the DocBook XSL stylesheets.  What version are you using?

I’m trying to use DocBook 4 with

To correct that problem, I changed

Now when I convert to PDF, I get

java.lang.IllegalStateException: 
  Flow 'xsl-region-body' does not map to the region-body in page-master 
  'blank'.  FOP presently does not support this.
