Making hyperlinks of page numbers in PDF index

Hi,

I'm using:
* Saxon 6.5.5
* Xerces 2.8.0
* Apache FOP 0.20.5

When I add an index to a book, the page numbers in the PDF version of
the index aren't hyperlinks. It would be nice if they were; has anyone
tried to make that happen?
