Docbook 5 and <cover>

Hello,

I'm using 

Docbook 5
Docbook XSL 1.73.2
xsltproc & FOP 0.95beta

to generate PDF.

I'm using the <cover> example from
http://www.docbook.org/tdg5/en/html/cover.html

to generate PDF coverpage,

However, I do not see the image or phrase/tagline/corner.

Any hints?
