Docbook 5 cover tag

Hello,

I'm using Docbook 5 with Docbook XSL 1.73.2, xsltproc & FOP 0.95beta to generate PDF output.

I'm using the <cover> example from
http://www.docbook.org/tdg5/en/html/cover.html

I do not see the image or phrase/tagline/corner on my PDF front page.

Any hints?
