Adding namespace to <html> in xhtml

Thread: Adding namespace to <html> in xhtml

Given a processing instruction of the form:
 
   <?import namespace="m" implementation="#MathPlayer"?>

The namespace "m" must have been defined in the <html> element (assuming
IE).

I can't find the place in the xhtml docbook stylesheets that would permit
adding namespaces to the <html> element.
