Hi Sam,
I'm not able to duplicate the error with the document you sent to me.  There 
are no duplicate ids that I can see.  I can't see why it is happening.

the output of
xmllint --xinclude --postvalid --noent impnotes.xml > xmllint.out
is in http://clisp.podval.org/xmllint.out.gz

it does not contain anything suspicious.
