Footnotes and Endnotes in chunked HTML output

Does anyone have any experience in generating endnotes in chunked HTML output. Basically, I would like to have all footnotes (including those in tables) to be displayed on one page at the end of the book with all footnote references linking to the relevant entry on that page. This would be similar to the way in which glossary terms work. 

 

There are some instructions about creating a customisation layer to do this at http://www.sagehill.net/docbookxsl/Endnotes.html but the instructions all relate to print output. The templates are quite different between fo/footnotes.xsl and html/footnotes.xsl 

 

Any assistance would be greatly appreciated.
