The html you are seeing is generated in chunk-common.xsl in the named template process.footnotes

Overriding that looks to be your answer.

I got that. I noticed that  in the output there is a BR and HR tag
separating the chapter from the footnotes. That is expected, but I
would also expect there to be some NOTES  title of some sort
immediately after the HR. if not, I'd like to add that.

I checked xhtml/footnote.xsl and I don't see any reference to an HR
tag. I'd like to add this HTML fragment (maybe h3 Notes /h3 ) where
the HR tag is added.

How would you do that?
