documentation added to schema

FYI, I finally got around to adding documentation to the schema. Please 
review and let me know if there are any issues!

If anyone is able to create the DTD version based on these, it would be 
greatly appreciated. I'm at XML 2009 this week and may not get to it 
until next week.
