Farrukh,

Better practice now is to simply use XSLT based approach.

The OASIS CPPA list has two examples - one my team developed at NIH, and the other Pim's team developed for applications in Holland.  They are broadly similar - but makes things much simpler - by using profiles approach - that are simply filled in via a web form or Word doc entered values.
