RE: OGC on-line Change Request form

This is the new OGC Change Request Form. It is in the process of being 
approved by the membership. It is actually just a derivative work based on the 
OGC Change Request Word document. We will also continue using the Word version. 
The whole reason for such a "rigorous" form is that it causes folks to think 
through a requirement and properly document a change requirement so that the 
revision team has the information necessary to make an informed judgment AND 
have the information necessary to actually make a suggested change of the 
revision team votes approval for said change.
