HAVE updated with HL7 ballot comments

Attached is the updated HAVE 2.0 WG04.  The HL7 ballot comments have been incorporated, with tracking on.  Two non-HL7 changes:  I happen to notice an extraneous word (which I deleted), and I added my name as an editor (I hope that is acceptable). 

 

Also attached is the HL7 ballot comment/reconciliation spreadsheet.
