Changes required to submitted documents

This Version & Previous Version - hyperlinks are incorrect & do not match text links. 

This Version: Change from /emergency/cap/v1.2/pr032/CAP-v1.2-PR023 to /emergency/cap/v1.2/pr03/CAP-v1.2-PR03

Previous Version: Change from /emergency/cap/v1.2/pr021{or12}/CAP-v1.2-PR021 to /emergency/cap/v1.2/pr02/CAP-v1.20-PR02

Latest Version - The .pdf hyperlink is incorrectly formed - Add .pdf.

Status - All three hyperlinks are incorrect. Need to replace the placeholder [TC Short Name] with the actual TC name.

Footer is incorrect - Change to the filename minus the extension.
