Bug in XML Catalog DTD

A correspondent has pointed out to me that the ATTLIST for the
catalog element contains "xmlns%s;", but that won't
work if the s parameter entity is overridden to be
not null.  We need another parameter entity like

	<!ENTITY nsattr "xmlns%s;">

and then we use the PEref "%nsattr;" in the ATTLIST.

Pkease fix.
